			    			The company offers a Flexible Spending Account (FSA) program that consists of the following 
			    			three plans:
			  			 
		        		Health Care Reimbursement Plan (HCR) provides a way 
                to pay for eligible out-of-pocket medical expenses with pre-tax dollars. 
			  			 
			    			Dependent Care Reimbursement Plan (DCR) provides a 
  							way to pay for dependent care expenses with pre-tax dollars.
			  			 
								Group Health Premium Plan (GHP) through payroll deduction - allows you to pay for your Group 
								Health premiums with pre-tax dollars.

 If you would like to enroll or submit a claim, Please select a form below: 
 
								¹ Print and mail the FSA Enrollment Form to your employer.
